Information on up to 600 patients of a NY medical practice was accessed by a third party under the direction of a then-associated physician to "solicit patients in connection with the physician's new employment."

An investigation indicated insider wrongdoing resulted in the access of certain health information (full name, date of birth, address, telephone number, e-superbills, appointment schedule).
